Ready to enroll?
You have two options for completing your enrollment; online and paper.
Click on the form of choice, fill it in, and submit it ASAP.
If you choose to submit your form via the online link an email verification will be required.
Please be sure to enter your email address correctly when prompted, and check your email for the verification link.
If you do not verify your email, your enrollment will be incomplete.
You will receive a copy of your documents once you verify your email.
Paper forms may be emailed to benefits@cowlesco.com or delivered to HR in person.
Medical, Vision, and Dental is available on the same form. Medical and vision are tied together.
Please note: Kaiser Permanente is available only in WA & ID.

When enrolling in your FSA, please remember that the total contribution amount is for the year and divided by 24 pay periods.

Your 401K is managed by Principal.

All full-time employees 21+ are automatically enrolled into our 401k, if you do not wish to participate, you must communicate that to us.
All full-time employees employed for a minimum of one (1) year receive employer paid term life, long term disability, and accident insurance.
After 90 days, you may enroll in Accident Insurance.
